BECOMING A DONOR:

A signed donor card is a record of your wish to donate, however, your next of kin is required to sign the consent form in order for donation to occur. Making your family aware of your wishes is absolutely necessary.

To become an organ donor, you can sign the back of your Alberta Health Care Card. If you have not signed the back of your existing card, and have had it laminated, you can find out how to obtain a new card here: http://www.health.alberta.ca/AHCIP/Q-replace-card.html. (The process took me about 30 seconds after spilling a cup of coffee on my un-laminated card).

A super rad donor card can also be found HERE. Just print it, sign it and keep that bad-boy in your wallet!

TO BECOME A LIVING DONOR:

To obtain information about becoming a living organ or tissue donor, you can contact the Human Organ Procurement & Exchange (HOPE) Program here:

NOTIFYING YOUR NEXT OF KIN:

A signed donor card is your record of your wish to donate. However, at the end of the day, it's your next of kin who makes the final decision. So be like, "Yo, mom. I wanna donate my organs. Kthx."
